Your Health in Loris, SC seeks a dedicated Clinical Scheduler to support our Care Manager role. You will coordinate in-person facilitator visits and provider encounters for your care team, ensuring timely scheduling and patient engagement across facilities, homes, and clinics.

The role emphasizes effective scheduling, coordination across disciplines, and proactive patient monitoring to support our value-based care model.

Qualifications

• High school diploma or equivalent required; healthcare education preferred.
• CMA/LPN/RN credentials are welcomed as a plus.
• Experience in medical or healthcare scheduling or admin roles.
• Familiar with medical terminology and insurance verification.

Responsibilities

• Register, verify and timely schedule patients per the care model.
• Track new patients and follow until scheduled.
• Schedule appointments according to clinical urgency and patient preferences.
• Optimize all discipline schedules and ensure coverage.
• Coordinate follow-ups after discharges from care facilities.
• Schedule TCM appointments within required windows.
• Maintain thorough documentation across systems.
• Interact with patients via email, portal, phone and in-person.
