The ideal candidate will coordinate with Directors of Population Health and care team members, handling scheduling across multiple systems while maintaining thorough documentation and high-quality patient communication.

Qualifications

• Strong scheduling experience or administrative background in healthcare.
• Familiar with medical terminology and insurance verification.

Responsibilities

• Register, verify and schedule patients according to care model.
• Track new patients and ensure they are scheduled.
• Schedule appointments based on availability and urgency.
• Optimize schedules across disciplines.
• Coordinate post-discharge follow-ups and timely care.
• Schedule Therapeutic/TCM appointments within required windows.
• Maintain thorough documentation.
• Utilize multiple software systems for tracking and follow-up.
• Communicate with patients via email, portal, phone, and in person to schedule and provide instructions.
