Job summary

HealthSource of Ohio is seeking a Care Manager RN to guide patients through the healthcare system, advocate and coordinate care after hospital admissions, discharge, and ER visits. The role emphasizes improving care continuity and reducing avoidable ER visits.

You will work in a not-for-profit community health setting with a focus on patient-centered care, chronic care planning, and participation in quality improvement initiatives.

Qualifications

• Current RN licensure in Ohio.
• 2–3 years of clinical RN experience.
• Outpatient RN experience and well-rounded clinical background.
• Experience with data analysis and health care operations.

Responsibilities

• Assists patients through the healthcare system as advocate and navigator.
• Coordinates continuity of patient care with patients and families after admission, discharge, and ER visits.
• Educates patients and families to reduce ER visits in the future.
• Creates and supports Chronic Care Plans.
• Participates in Patient-Centered Medical Home meetings and quality initiatives.
• Evaluates clinical care, utilization of resources, and development of new tools and procedures.

Do you have what it takes to be a Care Manager:

• Current RN Licensure in the state of Ohio
• 2 to 3 years clinical practice experience as an RN
• Well-rounded clinical experiences including work in outpatient settings as an RN
• Experience with data analysis and health care operations

A day in the life of a HSO Care Manager:

• Assists patients through the healthcare system by acting as a patient advocate and navigator.
