HealthSource of Ohio is seeking a Care Manager – Registered Nurse to support patients through the healthcare system as a patient advocate and navigator. The role focuses on coordinating care after hospital events and educating families on reducing ER visits, while contributing to chronic care plans and care-improvement initiatives.

The position emphasizes RN licensure in Ohio, outpatient experience, and data-driven operations to improve patient outcomes within a not-for-profit community health

Qualifications

• Current RN licensure in Ohio.
• 2–3 years clinical practice as an RN.
• Outpatient RN experience preferred.
• Experience with data analysis and health care operations.

Responsibilities

• Acts as patient advocate and navigator through the healthcare system.
• Coordinates continuity of patient care with patients and families after hospital events.
• Educates patients and families on reducing ER visits in the future.
• Creates and supports Chronic Care Plans.
• Participates in Patient-Centered Medical Home meetings and quality initiatives.

Do you have what it takes to be a Care Manager:

• Current RN Licensure in the state of Ohio
• 2 to 3 years clinical practice experience as an RN
• Well-rounded clinical experiences including work in outpatient settings as an RN

• Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
