Job summary

Qualifications

• Wisconsin Certified Social Worker or eligible to practice locally.
• 1+ year of experience with Family Care target populations.
• Ability to work in a fast-paced, team-based environment.

Responsibilities

• Conduct comprehensive assessments with IDT on member outcomes/needs.
• Coordinate care, services, and benefits for members.
• Develop and monitor member-centered plans with DHS compliance.
• Coordinate home visits and care conferences for long-term needs.
• Participate in 24/7 on-call rotation as required.
• Maintain accurate member records per DHS policy.
