Lakeland Care, Inc. is seeking a Care Manager to join our interdisciplinary team in Wisconsin.

The role focuses on care management and service coordination for frail elders and adults with disabilities, aligning services with DHS contract requirements and member outcomes. Reporting to the Care Management Supervisor, you will assess needs, develop member-centered plans, and monitor care across providers while maintaining HIPAA standards and fostering the member’s health, safety, and independence

Qualifications

• Certified Social Worker in Wisconsin with 1+ year experience in a family care population.
• Bachelor’s degree in Human Services with 1+ year experience or a bachelor’s in another field with 3+ years experience.

Responsibilities

• Perform comprehensive member assessments with an IDT to identify outcomes and needs.
• Coordinate care and services with providers to implement member‑centered plans.
• Educate members on rights and processes related to appeals and grievances.
• Participate in home visits and care conferences to reassess long‑term care needs.
