Job summary

Lakeland Care in Wisconsin is seeking an experienced RN Care Manager to join an interdisciplinary team serving frail elders and adults with disabilities. You will assess, plan, coordinate, and monitor care to promote health outcomes and quality of life across member plans.

You will conduct home and in-office visits, participate in 24/7 on-call rotations, ensure HIPAA compliance, and maintain accurate records while collaborating with providers, members, and families.

Qualifications

• Current Wisconsin RN license
• Bachelor's degree in nursing strongly preferred
• Two or more years of skilled nursing experience preferred, ideally in geriatrics/disabilities/home care
• Ability to access members' homes to provide care
• Ability to lift up to 25 lbs and hold a valid driver's license

Responsibilities

• Conduct comprehensive assessments and reassess as member outcomes change
• Coordinate acute and primary care services, care transitions and follow-up
• Develop, monitor and evaluate member-centered plans with cost and effectiveness in mind
• Promote prevention and wellness within care planning
• Maintain member records per DHS contract and Lakeland policy
• Participate in 24-hour on-call rotation and team meetings
