Qualifications

• Certified Social Worker in Wisconsin or equivalent licensure.
• 1 year minimum experience with Family Care populations.
• Computer literacy and data entry ability.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive assessment of member outcomes, needs and risks as part of IDT; reassess as conditions change.
• Coordinate acute and primary care services and transitions with RN Care Manager.
• Develop and monitor member-centered plans, balancing outcomes with cost and provider choices.
• Coordinate benefits and access to federal and state programs for members.
• Implement risk mitigation to promote health, safety and independence; respect member rights.
• Educate members on rights to appeal and participate in grievance processes.
