Job summary

Lakeland Care seeks a Care Manager to join an interdisciplinary team serving frail elders and adults with disabilities in Wisconsin. The role focuses on care management and service coordination based on member-centered plans to promote well-being.

Under the Care Management Supervisor, you will arrange services, monitor DHS contract requirements, and engage in home visits and care conferences to support long-term care needs.

Qualifications

• Wisconsin Certified Social Worker with 1+ year in Family Care populations.
• Bachelor's degree in Human Services with 1+ year in Family Care populations.
• Bachelor's degree in any field with 3+ years in Family Care populations.

Responsibilities

• Conduct comprehensive assessment of member outcomes, needs and risks.
• Collaborate with RN Care Manager to coordinate services and transitions.
• Develop, coordinate and monitor member-centered plans considering costs.
• Coordinate care and benefits to access federal/state programs.
• Participate in home visits and care conferences with the member and providers.
