Job summary

Lakeland Care Incorporated seeks a Care Manager to join an interdisciplinary team serving frail elders and adults with disabilities. The role focuses on comprehensive assessments and coordinating services per DHS contract requirements.

You will develop member-centered plans, coordinate care transitions, and monitor outcomes while educating members on their rights and options within the program. Strong collaboration and confidentiality are essential.

Qualifications

• Must hold Wisconsin Social Worker certification or have a relevant degree with experience in Family Care.
• Ability to access member homes and work in a fast-paced environment.
• Experience with government programs and DHS contract requirements preferred.

Responsibilities

• Conduct comprehensive assessments of member outcomes, needs and risks as part of IDT.
• Coordinate acute and primary care services, care transitions and follow-up care with RN Care Manager.
• Develop and monitor member-centered, outcome-based care plans and authorize services as needed.
• Coordinate benefits and access to federal/state programs for members.
• Participate in on-call rotation and continuous training to stay compliant.
