Job summary

RadNet, Inc. seeks a Care Manager Specialist to coordinate all aspects of a health home member's care in a fast-paced setting. You will track appointments, educate members, assist with consent forms, and engage with the care management team, providers, and families.

This role requires two years of healthcare or managed care experience, strong written and verbal communication, and proficiency with Microsoft Office Suite. Ability to multitask and maintain thorough documentation is essential.

Job description

The Care Manager Specialist has overall responsibility and accountability for coordinating all aspects of assigned health home member’s care. The Care Manager will track and arrange appointments, educate members and coordinate other aspects of the member’s health and community services. Responsibilities include but are not limited to: assisting members with consent forms, conducting outreach and engagement activities, conducting initial and periodic needs assessments, assisting with the development and execution of care plans, providing links to services and communicating among care management team, providers, members and families caregivers.

Additional Requirements
