Job summary

Samhsa is seeking a Care Manager Specialist to coordinate all aspects of a member’s care within the health home program. You will track appointments, educate members, and connect them with community services while supporting consent processes and engagement activities.

You will perform needs assessments, help develop and execute care plans, and liaise among the care management team, providers, members, and families to ensure comprehensive support.

Qualifications

• Must have minimum of two years’ experience in Healthcare, Managed Care setting or related field.
• Ability to perform in a busy, changing, multi-tasking work environment.
• Excellent written and verbal communication skills.
• Excellent interpersonal skills.
• Excellent organizational and documentation skills.
• Proficient in Microsoft applications.
• Proficient in Microsoft Office Suite (Outlook, WORD, Excel, PowerPoint) and database navigation required.

Job description

The Care Manager Specialist has overall responsibility and accountability for coordinating all aspects of assigned heath home member’s care. The Care Manager will track and arrange appointments, educate members and coordinate other aspects of the member’s health and community services. Responsibilities include but is not limited to: assisting members with consent forms, conducting outreach and engagement activities, conducting initial and periodic needs assessments, assisting with the development and execution of care plans, providing links to services and communicating among care management team, providers, members and families caregivers.
