Job summary

St.Vincent's Hospital Westchester is seeking a Care Manager Specialist to coordinate all aspects of an assigned health home member’s care in a busy hospital setting. You will track appointments, educate members, assist with consent forms, conduct needs assessments, develop care plans, and connect members with services, while coordinating with care management teams, providers, families, and caregivers.

Job description

The Care Manager Specialist has overall responsibility and accountability for coordinating all aspects of assigned heath home member’s care. The Care Manager will track and arrange appointments, educate members and coordinate other aspects of the member’s health and community services. Responsibilities include but are not limited to: assisting members with consent forms, conducting outreach and engagement activities, conducting initial and periodic needs assessments, assisting with the development and execution of care plans, providing links to services and communicating among care management team, providers, members and families caregivers.
