Job summary

Lakeland Care Incorporated in Wisconsin seeks a Care Manager to join an interdisciplinary team serving frail elders and adults with disabilities. You will conduct comprehensive assessments, coordinate services, and develop member-centered plans aligned with DHS requirements.

In collaboration with the IDT, you’ll monitor outcomes, facilitate care transitions, educate members on rights, and participate in on-call rotations to ensure high-quality, person-centered care.

Qualifications

• Certified Social Worker in Wisconsin with at least 1 year experience in Family Care populations.
• Bachelor’s degree in Human Services or related field with 1 year Family Care experience, or 3 years in non-Human Services with Family Care experience.
• Proficient with computers, typing and data entry.

Responsibilities

• Conduct comprehensive assessment of member outcomes, needs and risks and reassess as conditions change.
• Collaborate with RN Care Manager to coordinate acute and primary care services, transitions and follow-up care.
• Develop, monitor and evaluate member-centered plans considering cost and provider choice.
• Coordinate care and benefits to ensure access to federal/state programs.
• Educate members on their rights to appeal and grievance processes.
