Job summary

Molina Healthcare Inc. is seeking a care management professional to support LTSS activities and coordinate across a multidisciplinary team. You will work to ensure member progress toward outcomes while collaborating with caregivers, physicians, and support networks.

The role emphasizes timely assessments, waiver processing, and care plan optimization, with travel as needed and a focus on quality and cost-effective member care.

Qualifications

• 2+ years healthcare experience with LTSS or care management.
• Active license if required by state contract/ regulation.
• Bachelor's degree in healthcare (may be required by state).
• Valid driver’s license and reliable transportation for job-related travel.
• Strong communication with diverse populations and ability to adapt.
• Ability to work independently with minimal supervision.

Responsibilities

• Completes comprehensive member assessments within required timelines.
• Facilitates waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and care team.
• Monitors care plans and documents interventions, adjusting as needed.
• Promotes integration of LTSS and behavioral health services.
• Assesses medical necessity and authorizes waiver services.
• Facilitates ICT meetings for approval/denial of services.
• Educates and motivates members using motivational interviewing.
• Identifies barriers to care and coordinates psychosocial, financial and medical resources.
• Travel 25–40% may be required per state/contractual needs.
