Job summary

Molina Healthcare in Aurora, IL is seeking a Care Coordinator to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate member care across the continuum and progress toward outcomes, while aligning with quality and cost-effective care strategies.

You will complete member assessments, facilitate waiver enrollment, develop care plans, monitor progress, and coordinate services across behavioral health, LTSS, and community resources.

Qualifications

• At least 2 years health care experience, including LTSS exposure.
• Licensure/certification required only if mandated by state contract or regulation.
• Valid driver's license, reliable transportation, and adequate auto insurance for job travel.
• Knowledge of community resources and ability to work with diverse populations.
• Ability to work independently with minimal supervision and demonstrate proactive communication.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans with members, caregivers, physicians and others.
• Monitors care plan effectiveness and documents interventions and goal achievement.
• Promotes integration of LTSS with behavioral health and community resources.
• Assesses medical necessity and authorizes waiver services.
• Facilitates interdisciplinary care team meetings for approvals/denials.
• 25-40% local travel may be required.
