Job summary

Molina Healthcare is seeking a qualified care management professional to support LTSS care coordination and member outcomes. You will collaborate with multidisciplinary teams to ensure integrated delivery of care and compliance with waiver services and regulations.

Responsibilities include comprehensive assessments, care planning with members and providers, travel as needed, and proactive problem solving to address barriers to care while maintaining high-quality, cost-effective member care.

Qualifications

• 2+ years in health care with care management, managed care, LTSS exposure.
• Active unrestricted license in state of practice.
• Valid driver’s license and transportation for travel.
• Proactive, detail-oriented, able to work independently.
• Strong communication and relationship-building skills.
• Experience with diverse populations.
• Experience with substance use disorder settings is a plus.
• Proficient in Microsoft Office.

Responsibilities

• Complete comprehensive member assessments within timelines, including home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor care plans and adjust as needed.
• Promote integration of LTSS and behavioral health services.
• Assess medical necessity and authorize waiver services.
• Evaluate benefits and funding sources.
• Lead ICT meetings for approvals/denials.
• Educate and motivate members using motivational interviewing.
• Identify barriers to care and coordinate psycho/social/financial support.
• Identify critical incidents and develop prevention plans.
• Provide consultation to peers as needed.
• Travel 25-40% as required.
