The role focuses on comprehensive assessments, care planning, and monitoring of progress, while navigating waiver services, eligibility, and community resources to ensure quality, cost-effective member care.

Qualifications

• 2+ years healthcare experience with LTSS and care management
• LPN/LVN license active if required by state contract
• Bachelor's degree may be required depending on state contract
• Valid driver's license and reliable transportation
• Knowledge of community resources
• Ability to work independently with minimal supervision
• Excellent communication and problem-solving skills
• Proficiency with online portals and databases

Responsibilities

• Complete comprehensive member assessments within regulated timelines and conduct in-person home visits as required
• Facilitate waiver enrollment and disenrollment processes
• Develop and implement care plans with members, caregivers, physicians and care team
• Monitor care plan effectiveness and document interventions and outcomes
• Promote integration of services across LTSS, behavioral health, and community resources
• Assess medical necessity and authorize waiver services
• Coordinate ICT meetings for approval/denial of services
• Identify barriers to care and assist with psycho/social, financial, and medical obstacles
• Travel 25–40% as required by state/contractual requirements
