Job summary

Molina Healthcare is seeking a Care Coordinator to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate integrated member care across the continuum and help members progress toward outcomes with quality and cost-effective strategies.

The role involves conducting comprehensive assessments, coordinating waiver enrollment, developing care plans with members and providers, monitoring progress, and facilitating team meetings.

Qualifications

• 2+ years of health care experience including LTSS.
• Experience with care management or medical/behavioral health settings.

Responsibilities

• Complete comprehensive member assessments within regulated timelines and home visits as required.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and other health professionals.
• Monitor care plans to evaluate effectiveness and document progress.
• Promote integration of LTSS and behavioral health with community resources.
• Assess medical necessity and authorize waiver services.
• Coordinate interdisciplinary care team meetings for approvals/denials.
• Use motivational interviewing to educate and motivate change.
• Identify barriers to care and assist with psycho/social, financial, and medical obstacles.
• Identify critical incidents and develop prevention plans.
• Collaborate with licensed care managers as needed.
• Travel 25-40% may be required.
