Job summary

The role supports discharge planning, length-of-stay management, and continuous process improvement, requiring strong communication and clinical skills.

Qualifications

• Graduate of an accredited school of nursing (BSN preferred) or Masters Degree in Social Work (MSW) required.
• Two or more years clinical experience with one year in the acute care setting preferred.
• RN or LMSW in the state of employment is required for new hires.
• LBSW accepted for associates with 5+ years of demonstrated success and experience in CHRISTUS Care Manager I role.
• Certification in Case Management preferred.
• BLS preferred.

Responsibilities

• Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
• Coordinates integration of case management into patient care and discharge planning across departments and external service providers.
• Coordinates/facilitates patient care progression throughout the continuum of care in an efficient and cost-effective manner.
• Acts as patient advocate by negotiating resources with payors, agencies, and vendors.
• Documents in the medical record per regulatory and department guidelines.
• Assists with special projects and may serve as preceptor or orienter to new associates.
