Job summary

Intent Clinical seeks an Aging Care Manager to assess, plan, coordinate, and monitor care for aging clients in the Boston area. You will work with families and healthcare providers to ensure high-quality support in client homes, in the community, or remotely as needed.

The ideal candidate holds a Master’s in a related field, professional licensure, and at least 5 years of direct care experience in aging services, care management, or related settings, with excellent communication and a proactive

Qualifications

• Master’s degree in a related gerontology field or clinical discipline.
• Licensure in good standing (RN, NP, LMSW or equivalent).
• Minimum of 5 years direct care experience in care management or related settings.
• Strong knowledge of aging issues, healthcare systems, and community resources.
• Excellent communication and problem-solving skills; ability to work independently and in a multidisciplinary team.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive assessments and develop individualized care plans.
• Coordinate communication among clients, families, and healthcare providers; arrange services.
• Identify community resources and assist with navigation of benefits and entitlements.
• Perform regular follow-ups; adjust care plans as needed and document interactions.
• Provide family support, counseling, and guidance on long-term planning.
