Job summary

Intent Clinical is seeking an aging care manager to assess, plan, coordinate, and monitor care for aging clients in Fairfield, CT. You will work closely with clients, families, and healthcare providers to ensure appropriate support and services are delivered in the home, community, or remote settings.

The role requires a Master’s degree in Social Work, Nursing, Gerontology, or related field, active licensure, and at least five years of direct care experience.

Qualifications

• Master's degree in Social Work, Nursing, Gerontology, Occupational Therapy or related field.
• Professional Licensure in good standing (RN, NP, LMSW or other).
• Minimum of 5 years of direct care experience in care management, home care, acute care, and/or rehabilitation.

Responsibilities

• Assess, plan, coordinate, and monitor care and support services for elderly clients.
• Coordinate communication between clients, families, healthcare providers and support services.
• Identify community resources and assist navigating benefits and entitlements.
• Conduct regular follow-up visits and adjust care plans as needed.
• Provide family counseling and guidance on long-term care planning.
