Job summary

Crossroads Treatment Centers is seeking a Care Manager to connect with patients monthly, address their needs, and provide resources to support recovery. You will build relationships with community partners to expand patient resources and collaborate with a dedicated team to ensure high-quality care.

The role involves traveling to multiple outpatient locations, working 37–40 hours weekly with occasional evenings/weekends, and participating in community outreach events to promote addiction

Qualifications

• High School Diploma or GED required.
• Experience in fast-paced environments serving many clients preferred.
• Experience in healthcare, addiction medicine, community health, care coordination preferred.
• Experience with building individualized care plans is a plus.
• Knowledge of compliant documentation practices; Lived Addiction experience is a plus.

Responsibilities

• Connect with patients monthly to address needs and provide resources.
• Build and maintain relationships with community partners for patient resources.
• Work closely with a dedicated team to provide exceptional patient care.
• Care Managers work within a localized area with multiple locations and varying hours.
• Travel to primary and secondary work locations as required.
