Job summary

Crossroads Treatment Centers is seeking a Care Manager to support patients with opioid use disorder across multiple local locations. You will connect with patients monthly, coordinate resources, and collaborate with a team to deliver high-quality, evidence-based MAT options.

Travel to secondary locations is expected as part of the role. Ideal candidates have healthcare experience, care coordination skills, and knowledge of compliant documentation practices.

Qualifications

• High School Diploma or GED required.
• Experience in a fast-paced environment servicing many clients preferred.
• Experience in healthcare, addiction medicine, community health, care coordination preferred.
• Experience with building individualized care plans is a plus.
• Knowledge of compliant documentation practices.
• Lived Addiction experience is a plus.

Responsibilities

• Connect with patients monthly to address needs and provide resources.
• Maintain relationships with community partners for patient resources.
• Collaborate with a dedicated team to provide exceptional patient care.
• Participate in community awareness/engagement events to promote Crossroads and its mission.
