Job summary

Crossroads Treatment Centers is seeking a Care Manager to connect with patients monthly, address needs, and link them to resources. You will build relationships with community partners and collaborate with a dedicated team to deliver high-quality patient care.

Hours are part-time, with opportunities for community engagement events and travel within local locations. Experience in addiction care and compliant documentation is valued.

Qualifications

• High School Diploma or GED required.
• Experience in a fast-paced environment servicing many clients is preferred.
• Experience in healthcare, addiction medicine, community health, care coordination is preferred.
• Experience with building individualized care plans is a plus.
• Knowledge of compliant documentation practices.
• Lived addiction experience is a plus.

Responsibilities

• Connect with patients monthly to address needs and provide resources.
• Connect and maintain relationships with community partners for patient resources.
• Work closely with a dedicated team to provide exceptional patient care.
• Participate in community awareness/engagement events to promote Crossroads and our mission.
