Crossroads Treatment Centers is seeking a Care Manager Float for the Philadelphia area (N. Broad, NE Philly, Feasterville, and W. Tabor).

The role focuses on connecting patients with resources, building individualized care plans, and coordinating with community partners to ensure access to treatment and support services. The position requires a High School Diploma or GED, comfort in a fast-paced healthcare environment, and willingness to travel between primary and secondary locations to support

Qualifications

• Requires High School Diploma or GED.
• Experience in fast-paced environments servicing multiple clients is preferred.
• Experience in healthcare, addiction medicine, community health, or care coordination is preferred.
• Experience building individualized care plans is a plus.
• Knowledge of compliant documentation practices is required.
• Lived addiction experience is a plus.

Responsibilities

• Connect with patients monthly to address needs and provide resources.
• Maintain relationships with community partners for patient resources.
• Work with a dedicated team to provide exceptional patient care.
• Participate in community events to promote Crossroads and addiction treatment.
• Travel to secondary locations as required by district.
