The role emphasizes building relationships with community partners, participating in outreach events, and supporting patients with documentation and care planning in a fast-paced outpatient setting.

Qualifications

• High School Diploma or GED required; healthcare experience preferred.
• Experience in fast-paced environments servicing many clients.
• Healthcare/addiction medicine/community health/care coordination experience preferred.
• Experience building individualized care plans is a plus.
• Lived addiction experience is a plus.

Responsibilities

• Connect with patients monthly to address needs and provide resources.
• Maintain relationships with community partners for patient resources.
• Collaborate with a dedicated team to deliver quality patient care.
• Participate in community awareness events to promote Crossroads.
• Support patients in navigating services and resources.
