Job summary

Crossroads Treatment Centers is seeking a Care Manager to connect with patients monthly, coordinate care, and partner with community resources across our outpatient clinics. Travel to secondary locations is required, with primary assignments near the Clarion area.

You will work in a fast-paced healthcare setting, developing individualized care plans, documenting care, and engaging communities through events that promote addiction treatment and access to services.

Qualifications

• High School Diploma or GED required.
• Experience in healthcare, addiction medicine, community health, or care coordination preferred.

Responsibilities

• Connect with patients monthly to address needs and provide resources.
• Build and maintain relationships with community partners for patient resources.
• Collaborate with a dedicated team to deliver exceptional patient care.
• Participate in community awareness and engagement events to promote Crossroads and addiction treatment.
