Job summary

HomeWell Care Services, Inc. seeks a Care Manager to coordinate, implement, and supervise in-home services that align with the company’s mission and standards of care.

The role ensures regulatory compliance and proper client record maintenance across the agency’s activities. The position emphasizes assessment, care planning, supervision of care providers, and ongoing communication with clients and families to ensure high-quality service delivery.

Qualifications

• Two years of experience in home care, home health, or related field.
• Must have valid driver's license and auto insurance.
• Excellent written and verbal communication; able to interact with diverse audiences.
• Strong follow-up, attention to detail, initiative, problem-solving.
• Able to supervise and evaluate care providers and advocate for clients.
• Passionate about helping others live with dignity; present a positive, professional image.
• Must pass a criminal background check.

Responsibilities

• Complete Client Assessment packet for each new client and provide signed agreements.
• Develop and implement a written care plan with input from the client or representative.
• Collect fees for initial services at assessment or first day of service.
• Provide caregiver introduction and review care plan with client and caregiver; ensure binder is accessible.
• Supervise client care provided by agency personnel; ensure supplies and equipment on-site and within license scope.
• Regularly communicate with client or representative to follow up on service delivery and satisfaction.
• Perform scheduled and unplanned in-home client support visits.
• Conduct reassessments as needed due to client condition changes or events.
• Coordinate services when multiple providers are involved; offer referrals as needed.
• Serve as a resource for clients and families, facilitating education, empowerment, and quality of life.
• Investigate issues related to client care and well-being; document supervision and maintain records.
• Provide needed training and education to staff in client’s home or agency office.
• Assist with development and revision of client care policies and procedures.
