Job summary

HomeWell Care Services in Hartford region is seeking a Care Manager to coordinate and supervise the delivery of services for our aging clients, ensuring regulatory compliance and proper client records.

This role requires at least two years in home care, excellent communication, reliable transportation, and proactive problem solving to support families and caregivers with dignity and respect.

Qualifications

• Two years of experience in home care, home health, or related field.
• Must possess excellent written and verbal communication skills and interact effectively with diverse audiences.
• Must possess impeccable follow-up and attention to detail and be able to exercise initiative, problem solve, and apply sound judgment.
• Able to supervise, assess, and evaluate care providers and advocate for clients and families.
• Must pass a criminal background check.

Responsibilities

• Complete Client Assessment packet for each new client; provide a copy of all signed agreements to the client or designated client representative.
• Develop and implement a written care plan for each client with input and approval by the client and/or designated client representative.
• Collect fees for initial services at the time of assessment or by the first day of service.
• Provide caregiver introduction for first scheduled shift; review care plan with client and caregiver and set expectations for service delivery.
• Supervise client care provided by agency personnel.
• All necessary supplies and equipment are available on-site for safe client care.
• Document supervision of client care on the appropriate form and/or in the online management system in accordance with agency policies and procedures.
• Ensure client records are current and properly maintained in accordance with agency policies and procedures.
• Provide needed training and education to staff in the client’s home, via telephone and/or in the agency office.
• Assist with the development and revision of client care policies and procedures.
