Job summary

HomeWell Care Services CT299 is seeking a Care Manager to coordinate, implement, and supervise services for agency clients in alignment with our mission and standards of care. You will ensure compliance with regulatory requirements and maintain accurate client records, providing leadership in supervising care providers.

Responsibilities include developing care plans, coordinating services with other providers, communicating with clients and families, and driving quality of life improvements

Qualifications

• Two years of experience in home care, home health, or related field.
• Reliable transportation with current insurance and valid driver’s license.
• Excellent written and verbal communication skills with diverse audiences.
• Impeccable follow-up and attention to detail; able to exercise initiative and sound judgment.
• Able to supervise, assess, and evaluate care providers and advocate for clients and families.
• Passionate about helping others live with dignity; present a positive professional image.
• Must pass a criminal background check.

Responsibilities

• Complete Client Assessment packet for each new client and provide signed agreements to client or representative.
• Develop and implement a written care plan with input/approval by client or rep.
• Collect fees for initial services at assessment or first service day.
• Provide caregiver introduction for first shift; review care plan with client and caregiver.
• Supervise client care provided by agency personnel and ensure supplies/equipment are on-site and within scope of license.
• Regularly communicate with client or representative to ensure satisfaction and service delivery.
• Perform scheduled and unscheduled in-home client support visits.
• Conduct reassessments per agency frequency or changes in client condition.
• Coordinate services when multiple providers are involved in client care.
• Serve as a resource for clients and families; provide referrals as needed.
• Investigate issues related to client care and well-being; document supervision of client care in proper systems.
• Ensure client records are current and maintained per policies.
• Provide training and education to staff in client’s home or via agency office.
• Assist with development and revision of client care policies and procedures.
