Job summary

Community Care, Inc. in Sheboygan County seeks a Case Manager to collaborate with an RN Case Manager, perform assessments, develop care plans, coordinate services, and document adherence to standards.

Required a Bachelor's degree in Human Services or closely related field; 3 years of relevant experience may substitute. A valid driver's license and local travel are required; certification for long-term care functional screener is expected.

Qualifications

• Bachelor's degree in Human Services or closely related area.
• A minimum of three (3) years' experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity preferred.
• Eligibility for long-term care functional screener certification is required.
• Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Responsibilities

Case Manager Job Responsibilities:
