Job summary

Community Care, Inc. in Ozaukee County, WI is seeking a Case Manager to join a mission-driven team serving older adults and adults with disabilities. The role supports community-based care planning, assessment, and coordination, with a Monday–Friday schedule and a $5,000 sign-on bonus.

This position emphasizes collaboration with an RN Case Manager, documentation accuracy, and adherence to established care standards while helping members live independently in the community.

Qualifications

• Bachelor’s degree in a Human Services or related field is required.
• Minimum of three (3) years’ experience with seniors or adults with disabilities may substitute for a non-human services degree.
• Eligibility for long-term care functional screener certification is required.

Responsibilities

• Manages an assigned caseload of program members in collaboration with an RN Case Manager.
• Completes comprehensive member assessments, member-centered care plans, and functional screens.
• Provides ongoing care coordination and quality assurance monitoring of services.
• Maintains required documentation in adherence to standards.
