Job summary

Actalent in Elkhorn, WI is seeking a Care Manager to provide comprehensive case management and care coordination for elderly individuals and adults with disabilities, helping them live as independently as possible in the community. This full-time, hybrid role partners with an RN Care Manager and a team to deliver person-centered plans.

Responsibilities include member assessments, care planning, advocacy, and collaboration with nurses and social workers, with local travel within Rock County to

Qualifications

• Bachelor’s degree in Human Services or closely related area.
• 1+ year of experience working with the elderly population.
• Case/Care management experience preferred.
• Medicare experience preferred.

Responsibilities

• Manage an assigned caseload with RN support, ensuring timely service delivery.
• Complete comprehensive member assessments (physical, social, psychosocial).
• Develop and maintain member-centered care plans to promote independence.
• Perform functional screens and document results as required.
• Provide ongoing care coordination and adjust services as needs change.
• Monitor service quality and address gaps or improvements.
• Maintain documentation in line with standards and policies.
• Provide advocacy, resource linkage, and psychosocial support.
• Collaborate with nurses, social workers, and other care professionals.
• Coordinate care within Medicare, Medicaid, and BadgerCare programs.
• Engage with members, families, and providers to ensure continuity of care.
• Attend team meetings and contribute to care planning.
• Travel locally within Rock County for home/community visits.
• Adhere to organizational policies and professional social work guidelines.
