Qualifications

• Graduate of Nursing Program required.
• Master's degree preferred.
• 2 years of experience in an acute or post-acute setting required.
• RN - Georgia licensure required; NLC/eNCL multistate licensure acceptable.

Responsibilities

• Accountable for expediting timely and safe discharge for all patients in the care load.
• Collaborate with interdisciplinary teams to ensure safe transitions of care.
