Job summary

WVUH West Virginia University Hospitals is seeking a seasoned Care Manager to oversee discharge and transition planning performed by the Care Management team in Morgantown, WV. You will audit activities for accuracy and lead a multidisciplinary approach to high-risk cases.

You will conduct weekly Clinical-Financial meetings, ensure compliance with regulatory requirements, and mentor staff through orientation and ongoing development, while fostering collaborative relationships across departments.

Qualifications

• Requires Bachelor’s Degree in Nursing or Master’s Degree in Social Work.
• Must hold one of the current licenses as required by state boards (RN, LGSW, LCSW, LICSW, LCSW in PA).
• Two years of Care Management or supervisory experience preferred.

Responsibilities

• Oversees discharge/transition planning by Care Management staff and audits activities for accuracy.
• Leads weekly Clinical-Financial meetings to monitor high-risk patients and collaborates with multidisciplinary teams.
• Keeps current with regulatory requirements and insurance coverage related to care delivery in acute and non-acute settings.
• Participates in orientation and evaluation of new staff with Preceptor.
• Serves as a clinical resource and role model for department personnel.
