The role focuses on ensuring member progress toward outcomes, coordinating care plans, and promoting service integration, with travel required in some states based on contractual requirements.

Qualifikationen

• At least 2 years health care experience, preferably in behavioral health, or equivalent education/experience.
• Licensed behavioral health clinician: active license in state of practice.
• Valid and unrestricted driver's license for job-related travel.
• Experience with severe mental health concerns and substance use disorders.
• Knowledge of whole person care, chronic conditions, and discharge planning.
• Data entry skills and experience with a clinical platform.
• Excellent verbal and written communication skills.
• Proficiency with Microsoft Office suite.

Aufgaben

• Completes comprehensive behavioral health assessments and determines eligibility for care coordination.
• Develops and implements care plans with member, caregiver, and healthcare professionals.
• Conducts telephonic, face-to-face, or home visits as required.
• Monitors care plan effectiveness and documents interventions and outcomes.
• Maintains ongoing member caseload and regular outreach.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and clinical guidelines to support change.
• Assesses barriers to care and provides coordination assistance.
• May provide consultation to peers as needed.
• 25-40% local travel may be required.
