Aspire Health Partners is seeking a Care Manager to deliver exceptional customer service, assist clients with psychosocial barriers, and ensure continuity of care across programs. You will navigate Aspire's system of care and coordinate with community services to support individuals and families in need.

Responsabilidades

• Respond to and provide short-term intervention services for individuals experiencing mental health-related crises in the community.
• Connect individuals in crisis to appropriate behavioral health, substance use, and community-based services and supports.
• Conduct care management services for individuals who meet substance use diagnostic criteria, DLA-20, and ASAM assessment requirements.
• Receive and manage referrals from the Access Department, Mobile Response Teams, and Crisis Receiving Center (CRC).
• Deliver services using a trauma-informed, person-centered approach while ensuring a high-quality customer experience.
