Job summary

Aspire Health Partners in Orlando, FL, is seeking a Case Manager to assess needs of chronically mentally ill adults and coordinate services. You will screen, refer, and manage emergencies within a clinical team, document care plans, and advocate for clients at community meetings.

Requires a Bachelor's degree in a related field and Level II background clearance. Pay rate is $19.95/hour with benefits including medical, dental, vision, PTO, and retirement options.

Qualifications

• Bachelor's Degree in Social Work, Psychology or Human Services related field.
• Level II Background clearance required; Aspire covers fingerprinting costs.
• Acceptable Motor Vehicle Registration driver's license as per Aspire insurance policy.

Responsibilities

• Providingscreening, referral, emergency, crisis and case management services,participating in clinical treatment team.
• Conducting intakeassessments to determine family needs, efficiently documenting all activities,conducting activities with the client and families, providing regular input tothe treatment plan
• Attendingcommunity meetings to advocate for client needs, providing support and crisisintervention when necessary
• Documentingclinical records and additional program specific documents while maintainingthe integrity of the client.
• Ability to useprogram technology to communicate effectively with staff
• On occasion, actas the authorized client transporter for necessary treatment and services.
• Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.
