Job summary

Aspire Health Partners in Florida is seeking a Case Manager to assess and address the needs of chronically mentally ill adults. You will work within a collaborative team to connect clients with necessary services and resources, coordinating care across multiple settings to support recovery and independence.

Qualifications

• Bachelor's Degree in Social Work, Psychology or Human Services related field.
• Level II Background clearance required (Aspire covering fingerprinting costs).
• Acceptable Motor Vehicle Registration driver's license per Aspire's insurance policy.

Responsibilities

• Providing screening, referral, emergency, crisis and case management services, participating in clinical treatment team.
• Conducting intake assessments to determine family needs and documenting activities.
• Attending community meetings to advocate for client needs and provide crisis intervention when necessary.
• Documenting clinical records and program-specific documents while maintaining client confidentiality.
• Using program technology to communicate with staff.
• Acting as authorized client transporter when needed.

Job Purpose:

As a Case Manager at Aspire Health Partners, your job purpose to provide overall assessment and needs of chronically mental ill adults.

Job Functions:

• Providingscreening, referral, emergency, crisis and case management services,participating in clinical treatment team.
• Conducting intakeassessments to determine family needs, efficiently documenting all activities,conducting activities with the client and families, providing regular input tothe treatment plan
• Attendingcommunity meetings to advocate for client needs, providing support and crisisintervention when necessary
• Documentingclinical records and additional program specific documents while maintainingthe integrity of the client.
• Ability to useprogram technology to communicate effectively with staff
• On occasion, actas the authorized client transporter for necessary treatment and services.
• Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.

Qualifications

• Bachelor's Degreein Social Work, Psychology or Human Services related field.
