Job summary

Tri County Care Management Organization Inc. is seeking a full-time Care Manager based in our Branchburg, NJ office to support youth facing behavioral, emotional, social, developmental, and mental health challenges.

You will coordinate intensive wraparound services to help youth thrive at home, school, and in the community. Responsibilities include crisis planning, ISP development, resource sourcing, and acting as liaison among families, providers, schools, and courts.

Qualifications

• Bachelor’s degree in social work, psychology or related discipline.
• At least 1 year of experience in a child service system.
• Strong interpersonal and organizational skills.

Responsibilities

• Prepare and carry out short-term crisis plans and ISP as needed.
• Source resources for youth to access formal and informal services.
• Act as liaison between families and providers.
• Travel to meetings at homes, schools, courts, and attend other services with youth and families.
• Advocate for youth and families in meetings as needed.
• Maintain electronic health records and youth files.
• Participate in required training opportunities.

Are you passionate about mental health and helping children and families succeed?Tri County Care Management Organization (TCCMO) is a non-profit organization that provides care management (case management) services for children with complex challenges in Hunterdon, Somerset and Warren Counties.We are currently seeking a Care Manager based in our Branchburg, NJ office to provide support to youth who are experiencing behavioral, emotional, social, developmental, and/or mental health challenges.
