Join MonmouthCares as a Full-Time Care Manager and make a difference in the lives of individuals in need. This hybrid role offers flexibility to work from home, with local eligibility in Monmouth County, NJ. All family visits are conducted in the community.

Responsibilities

Your day-to-day as a Care Manager includes meeting with families to assess their needs and coordinate care for children and youth facing mental health, behavioral health, and intellectual/developmental challenges. You will identify the best resources and services to support these individuals, ensuring they receive necessary care, and collaborate with families and community partners to contribute to a stronger, more supportive environment.

What matters most

We require a background as a social worker in social services or human services, with attention to detail and familiarity with deadlines. The role requires reliability, responsibility, and accountability in duties, ensuring high quality care for children and youth. A comprehensive knowledge of the educational system, including special education, and familiarity with child welfare, juvenile justice, and child protection services systems. Minimum of one year experience with at-risk children and families.
