Job description

Position Purpose: Supports administrative care management activities including outreach, answering inbound calls, and scheduling services. Acts as a point of contact for members, providers, and staff to resolve issues and documents member records in accordance with current state and regulatory guidelines.

  1. Provides outreach to members via phone to support care plan next steps, community or health plan resources, questions or concerns related to scheduling, and ongoing education for both the member and provider throughout care/service.
  2. Supports members by connecting them to health plan and community resources to ensure high-quality customer care.
  3. Applies working knowledge of assigned health plan activities and resources.
  4. Serves as the front-line support for member and provider inquiries, requests, or concerns, including explaining care plan procedures and protocols.
  5. Supports member onboarding and handles administrative duties such as sending welcome letters, correspondence, and educational materials to facilitate successful member/provider relationships.
  6. Documents and maintains non-clinical member records in compliance with standards and policies, and provides information to providers as needed.
  7. Knows existing benefits and local resources, and makes referrals to address Social Determinants of Health (SDOH) needs.
  8. Performs other duties as assigned and ensures compliance with all policies and standards.

Education/Experience: High School diploma or GED required. 1–2 years of related experience preferred.

Pay Range: $17.50 - $27.50 per hour

Centene offers a comprehensive benefits package including competitive pay, health insurance, 401K and stock purchase plans, tuition reimbursement, paid time off plus holidays, and flexible work options (remote, hybrid, field, or office schedules). Actual pay will be adjusted based on skills, experience, education, and other factors permitted by law. Total compensation may include additional incentives.
