Job summary

Hope Services, LLC is seeking a full-time leader to supervise the Care Management Team, promoting integrated, whole-person care for children, adolescents, and families. You will provide clinical and administrative oversight, ensuring service quality and compliance while guiding a diverse team.

Responsibilities include recruiting and training staff, conducting performance reviews, leading case conferences, and ensuring timely documentation.

Qualifications

• Licensed professional in a related field (LCSW/LCMHC/LPA/LMFT/RN).
• Minimum 3 years of care management or care coordination experience.
• Experience with co-occurring behavioral health and I/DD is preferred.

Responsibilities

• Provide clinical and administrative supervision to a care management team of up to 8 Care Managers and 3 Extenders.
• Lead staff recruitment, onboarding, training, performance management, and related processes.
• Ensure quality service delivery and compliance through record audits and case conferences.
• Participate in home visits and Child and Family Team (CFT) meetings.
• Ensure timely and accurate documentation of all Care Management activities.
