Job summary

Qualifications

• Licensed as LCSW, LCMHC, LPA, LMFT, or RN.
• Minimum 3 years in care management or care coordination.
• Experience with co-occurring Behavioral Health and I/DD preferred.

Responsibilities

• Provide clinical and administrative supervision to a care management team of up to 8 Care Managers and 3 Extenders.
• Lead staff recruitment, onboarding, training, performance management, discipline, and termination processes.
• Ensure quality service delivery and compliance through activities such as case conferences, record audits, observing team performance, and participation in home visits and CFT meetings.
