Job summary

UHC United Hospital Center in Bridgeport, WV seeks a care management leader to oversee discharge/transition planning, audit activities, and collaborate with multidisciplinary teams to improve patient flow and resource utilization. The role requires five years of nursing or medical social work experience, with two years in inpatient settings and two years in care management or supervision; RN and social work licensure are preferred.

Qualifications

• Bachelor's Degree in Nursing or Master's Degree in Social Work.
• Must possess one of the following current licenses as required by state board where services will be provided.
• Current Registered Nurse license issued by the state in which services will be provided or current multi-state RN license through eNLC.

Responsibilities

• Oversees discharge/transition planning and audits activities for accuracy of delivery.
• Conducts weekly Clinical-Financial meetings to monitor patients with high dollar, high LOS, or social barriers.
• Promotes quality of care through collaboration with all service team members.
