Qualifications

• Bachelor’s Degree in Nursing or Master’s Degree in Social Work.
• Current RN license issued by the state or multi-state license via eNLC.
• WV licensure options: LGSW, LCSW, or LICSW in WV; LCSW/LSW in PA.

Responsibilities

• Oversees discharge/transition planning by Care Management staff.
• Conducts weekly Clinical–Financial meetings to monitor high-cost or high-barrier cases.
• Collaborates with multidisciplinary team to problem solve complex cases.
• Ensures compliance with regulatory requirements and payer policies in care delivery.
• Provides guidance and feedback to staff and leadership on performance and development.
• Promotes interdepartmental communication and maintains quality patient care.
