Job summary

Regions Hospital is hiring a Hospital Care Management Supervisor. As an integral member of the interdisciplinary healthcare team, this position contributes to collaborative patient care and offers education to healthcare professionals on the psychosocial aspects of treatment.

This role supports the department manager in planning, development, coordination, and oversight of services to ensure efficient day-to-day operations while providing leadership and consultation to program staff.

Qualifications

• RN licensure in Minnesota or LICSW licensure in Minnesota
• Minimum of 5 years experience in your profession
• Experience leading or mentoring teams is preferred

Responsibilities

• Mentor and coach staff, support department leaders, provide performance feedback
• Conduct clinical and psychosocial assessments, develop care plans, and refer patients/families
• Utilize a comprehensive range of clinical approaches to patient care
