Responsibilities

• Evaluate member needs, barriers to care, and resources; facilitate and coordinate care plans.
• Develop or contribute to personalized care/service plans and connect members with providers and community resources.
• Provide psychosocial support and educate members and caregivers on procedures, benefits, and services.
• Coordinate between member/family and care team to ensure timely access to services.
• Document and maintain all member information to ensure regulatory compliance.
