Job summary

Responsibilities

• Evaluates member needs, barriers, and resources to tailor care plans.
• Develops or contributes to personalized care plans and coordinates providers.
• Provides psychosocial support and assists access to local resources.
• Coordinates between member, family, and care team for timely services.
• Monitors progress toward goals and revises plans as needed.
• Maintains member information to ensure regulatory compliance.
• May perform on-site visits and educate on procedures and benefits.
• Other duties as assigned and compliant with policies.
